The Care Coordinator delivers innovative, proactive and
compassionate service that exceeds our member's expectations. The
Care Coordinator is the knowledgeable and trusted advocate for
Prominence members, as well as represents facility and its policies
and procedures when assisting customers through initial contacts,
education opportunities, inquiries, problems or complaints. The
Care Coordinator provides effective two-way communication to ensure
quality services and appropriate problem resolution for all parties
involved. The main focus of this position is contacting Prominence
Health Plan Medicare members by phone to inform and educate them on
the health insurance offerings, products or services, as well as
respond to inquiries and assist with quality improvement and member
health assessments. The Care Coordinator will work with Senior Care
Coordinators, other Prominence Department members and health care
providers, on helping members develop realistic and attainable
mental and/or physical health goals, and assist members in
identifying needed resources to help accomplish member goals.
Bachelor's degree in Community Health, Public Health, or related
Minimum 3 years' experience in customer service or similar
public contact work related to communication, problem-solving and
Valid Driver's license, applicable insurance and reliable
Ability to effectively communicate in English, both verbally and
Empathetic, Caring, Compassionate Listener.
Excellent computer skills.
Able to work with a diverse multicultural and socioeconomic
Familiarity with health care delivery and/or health insurance
Knowledge of medical terminology.
Ability to prioritize and multi-task.
Strong problem-solving and critical thinking skills.
Experience working with the Medicare population segment
Ability to travel local and regional up to 20 to 30% of the
